Parking Brake Adjusting Screw Failure

The parking brake of a flatbed tow truck failed and allowed the truck to roll into traffic causing a collision. Preliminary investigation revealed a fractured adjusting screw within the parking brake lever assembly. Subsequent evaluation was performed to determine the cause of the adjusting screw failure.
