A review of factors leading to the catastrpohic failure of fasteners on a wheel assembly

During maintenance operations, multiple fasteners connecting a multi-piece wheel assembly on a piece of heavy equipment catastrophically failed, leading to the separation of the wheel components. This presentation will review the contributing factors that led to this near fatal event.
