While those in the field of failure analysis tend to focus on the specialized mechanisms of a failure related to materials science & engineering, a convincing case can be made that an overwhelming majority of engineering failures, have their true root causes sourced from either a single instance, or a sequential series of, mostly avoidable, mis (or even "non-") communication events. The author will present various case studies, both infamous and mundane from his career experiences where the failure in question could likely have been avoided if only efficacious communication had been employed between the sender and receiver of an engineering-related message. Any emerging professional who gains an appreciation and mindfulness of such wisdom, gained through painful life experiences of the author and humanity in general, will be better prepared in their forthcoming career development than those lacking such awareness.
