Examination of a Helicopter Engine to Transmission Driveshaft that Fatigue Cracked

A helicopter main driveshaft cracked as a result of fatigue that initiated at an oversized subsurface inclusion cluster. The inclusion was located just below the nitride layer and the maraging steel had an aluminum-ceramic coating. The fail-safe mechanism successfully engaged and the helicopter was able to land safely.
