Metallurgical evaluation of a jet engines high pressure compressor components after an in flight event

A jet engine used in a non-commercial application experienced multiple stalls during a flight. Post-flight inspection found multiple fractured high pressure compressor (HPC) components. A metallurgical evaluation determined the root cause was a misinstalled bolt bushing in the HPC assembly. The findings led to a design change to account for human factors during installation.
