A stainless-steel tee with a polyvinylidene fluoride (PVDF) liner experienced a leak that sprayed bleaching fluid throughout an industrial facility. The tee was analyzed to determine the possible cause(s) of failure. The investigation explores how multiple failure mechanisms such as dehydrofluorination, osmotic blistering, and slow crack growth (SCG) worked together to lead to eventual failure of a material known for its resilience to environmental degradation.
