Failure Analysis Flow Decision Tree

The process of Failure Analysis (FA) seems obvious; find the Root Cause of the failure. How to go about resolving the root cause of the failure is not as obvious. This presentation will describe the process of FA from discovery of the failure through to determination of the Root Cause on to recognition of work well done. As each step is discussed the process for that stage will be detailed with a brief description of the associated equipment. As analysis progresses there are points where decisions must be made that are irreversible. The consequence of each decision at each stage within the FA process will be presented. By understanding the consequences of each decision good FA processes can be developed that limit the jeopardy of each decision. This presentation would be applicable for any new analyst. Experienced analysts who have focused on one aspect of the process such as deprocessing, electrical test or fail site isolation would benefit by expanding their scope of understanding of the overall FA process. Managers who have been analysts who wish more breadth of understanding of the FA process would benefit from this presentation.
