A Review of the Findings and Recommendations of the Columbia Accident Investigation Board

The Columbia Accident Investigation Board (CIAB) determined that the physical root cause of the Space Shuttle Columbia accident was determined to be a breach in the Thermal Protection System on the leading edge of the left wing. The findings and recommendations of the CIAB covered areas that included the physical cause as well as human causes such as training and scheduling and latent causes such as organization. This presentation will cover these areas and make a comparison of this accident to another accident that was equally shocking to the world.

Summary: The Findings and Recommendations of the Columbia Accident Investigation Board will be reviewed and compared to another accident.
