A series of failures relating to heat treatment specifications are presented. This survey includes failures resulting in overload and fatigue fractures, statically and cyclically loaded components. The cases reviewed include rotating shafts and engine cylinder rods. The importance of proper heat treatment specification and implementation is addressed.
