Lewy body dementia and related disorders

Dementia with Lewy bodies (DLB) and Parkinson's disease with dementia (PDD) are emerging as two related disorders that can confront the clinician with significant difficulties in diagnosis and management. This symposium will introduce the concept of a spectrum of Lewy body disorders that share a common pathological and neurochemical basis, but may present with a wide varietyof clinical manifestations including extrapyramidal motor symptoms, cognitive and neuropsychiatric features and autonomic dysfunction. Lewy body disorders are characterised pathologically by aggregates of a synaptic protein, alpha-synuclein, the structure, metbolism and function of which are being intensively studied in animal models, since the discovery that mutations in the alpha-synuclein gene may lead to LB disease. The specific pattern of neuropsychological deficits of DLB not only may aid in differential diagnosis from other types of dementia but also may assist understanding the genesis of core clinical features such as visual hallucinations. Finally, a structured approach to the management of DLB and PDD will be presented.
