Background: The People’s Republic of China has the world’s largest and most rapidly demographically aging population. By 2030 it is forecast that 24% of all Chinese, 400 million persons in all, will be aged 65 years and over. Up to 10 million of them may suffer from dementia. Specialized health services and welfare for older persons are as yet rudimentary, with a heavy reliance on family care.

Methods: The 10/66 population based study in China will recruit 2000 persons aged 65 years and over; 1000 in West Beijing a central city district close to the Forbidden City, and 1000 in Daxing a traditional rural area around 100 kms out of Beijing City. There are profound differences between city and rural populations. For example, in Beijing City it is estimated that up to one third of older persons live on their own, whereas in Daxing it is anticipated that all will live with their children.

Objective: The design of the 10/66 population based study in China will be presented in the context of previous epidemiological research in Beijing and other regions in China. The focus will be upon the potential of the new studies for extending the knowledge base beyond that from previous research.
