S081-003 Older Adult Wraparound

Wraparound is a System of Care that includes a definable planning process involving older adults and their adult family members, care givers, significant persons, and service providers that results in a unique set of community services and natural supports individualized for that older adult and family in order to achieve a positive set of outcomes. Originally a mental health model that was developed to serve at-risk children and their families, this Adult WRAP model was adapted out of the principles which govern the child model after a 5 year federal grant demonstrated the value of this approach. In our older adult population we were frequently seeing clients whose one or other or many needs were 'falling through the cracks' among service providers and /or family members. As the mental health provider we were concerned that the client's depression, for example, would not lift as long as there were other substantial obstacles to meeting needs that were not traditionally within the purview of mental health agencies. Adult Wraparound is a team approach that addresses these needs of the older adult by using the strengths of the adult, the family, the neighborhood and the community at large.
